Post Acute Care

Our clinical team will perform home visits at scheduled intervals to minimize complications and facilitate rapid recovery after a major illness requiring hospitalization. You may be sent home with new medications and/or wound care instructions upon leaving hospital after a surgery or cardiac event. Early proactive clinical monitoring can mitigate complications and prevent re-hospitalization.

Our recovery programs focus on the following:

• Wound Care
• Post-Operative pain management
• Optimum medication dosing
• Adverse drug reactions and medication side effects
• Mobilization
  
  

We are also able to facilitate enhanced care giving, including purchasing up to 24 hours of continuous care if required.
This is available as part of the ADVOCARE and WELLCARE programs.
